Thank you for choosing to participate in the National Institutes of Health / National Cancer Institute study on cancer pain. We hope this app makes it easy for participants like you, to complete study check-ins wherever you may be located.

Using this app: You will be asked to come to the NIH Clinical Center between two and four times while you are participating in this study. You will also be asked to check-in at-home approximately 3 times per week during the study. The assessments you will take part in at the Clinical Center (in-clinic) or remotely (at-home) will take approximately 3 minutes to complete.
· During these check-ins you will be asked to:
· complete a questionnaire in an application using a mobile phone,
· record a video of yourself reading an approximately 15 second passage of text, and
· record a video of yourself responding to an additional question(s).

You will be compensated for your participation as long as you complete at least five or more submissions. You will receive $15 per week in which you submit three or more questionnaires, including video recordings, completed at-home. You will receive an additional $15 for each completed series of video recordings and questionnaires completed in-clinic. You will be paid a maximum of $225 for the duration of this study. You will not be paid until you have completed the study. No compensation will be provided if you complete five or less submissions.
